About insurance

・When driving a bicycle within Saitama Prefecture, it is now mandatory to take out bicycle damage insurance. Please take out liability insurance before participating.

・The information received from customers will be used only for holding events and providing information, and will not be used for any other purpose.

- Information received from customers will not be provided or disclosed to third parties unless required by law or with the customer's prior approval or consent.

・We will strive to prevent leakage of customer data acquired and stored by our company, and will manage it with all possible care.

To ensure safety

・Please be sure to wear a helmet while cycling.

・In the unlikely event that you are injured and need to go to a medical facility, you will need your insurance card, so please bring it with you.

Agreement

・I acknowledge that I am paying close attention to my health and that I am in good physical and mental health. Also, if you are not feeling well on the day of the event, you will be asked to decline participation.

・If the applicant is under 20 years of age, this consent form will be deemed to have been given by the applicant's parent or guardian.
